AUTHORIZATION, CONCUSSION, WAIVER & RELEASE
I hereby apply for a reservation and give my child permission to attend an event hosted by Mike Farley’s Kickers Camp and MaxOut Sports Testing, LLC (a.k.a “Northwoods Combine”) My child and I have read and understand the Northwoods Combine’s procedures for discipline, applications, deposits and refunds.

PHOTOS/VIDEOS CLAUSE:
I understand that the Northwoods Combine may take photographs and videos of the camp participants and agree that the Northwoods Combine shall be the owner of these photos and videos. I also acknowledge that these photos and videos may be used to promote future combines and relinquish all rights I may claim.

CONCUSSION:
I understand and have read the Concussion and Head Injury Information sheet and understand what a concussion is and how it may be caused. I understand that Northwoods Combine participants must be removed from the combine if a concussion is suspected and that it is my responsibility to seek medical treatment. I understand that anyone suspected of a concussion cannot return to practice/play until providing written clearance from an appropriate health care provider to Northwoods Combine. I understand the possible harmful consequences of a participant suspected of a concussion returning to practice/play too soon.

HOLD HARMLESS:
If your child or ward will be under the age of 18 years while attending the Northwoods Combine, it is Northwoods Combine policy to secure your consent of treatment in the event of an illness or injury, therefore...

I give consent for medical treatment and permission to attending physician to hospitalize, secure proper treatment and order injections, anesthesia, or surgery. I will be responsible for any medical treatment or other charges in connection with my child’s participation at and will indemnify Northwoods Combine as to any such charges. I certify that within the past two years, my child has had a physical examination and that as of now, is physically able to participate in football activities that may involve, among other things, physical contact of the body with other persons or objects, including the ground and the effects of weather conditions and that at the Northwoods Combine, my child may incur a risk of injury. My child and I assume that any such risk my child, and I (for myself, my spouse, my child and our respective heirs) agree to hold harmless, indemnify and forever waive, give up and release the Northwoods Combine, its owners, employees, coaches, directors, agents and staff from any and all liability for any claim for damages which I, or my child, may have for injuries, damages, costs, expenses, illness or death which are sustained, incurred or required in connection with the Northwoods Combine.
